Undergraduate Program Forms

Listed below are the various forms relevant to the undergraduate program in computer science. These are all PDF documents, and you will need the appropriate software to display or print them.

Matriculation

This form is used to summarize progress toward your degree. Prepared by your academic advisor, it records the courses you have taken and those you still need to take.

Concentrations

If you are pusuing one of the available undergraduate concentrations, then this form (prepared by your academic advisor) will show the courses required and which ones you have completed. It is included with the matriculation form.

Computer Science Minor

This form summarizes your progress toward the completion of a minor in computer science.

Core Extension

Use this form to propose the set of courses you wish to use in your core extension. The selected courses must be approved by the undergraduate program committee.

Independent Study

This form is used to propose a topic for an undergraduate independent study course (CSCI 4990). It requires identification of the faculty who will supervise the independent study, and should be prepared in consultation with that person. Approval of the independent study by the undergraduate program committee is required prior to registration.

Internship Agreement

This form is used to specify the details associated with an undergraduate internship (CSCI 4950). It also supplies information about the methods that will be used for evaluating the internship.

Internship Evaluation

This form is completed by the individual supervising an intern in CSCI 4950 to report an evaluation.
